Alcohol-Related Licensing Materials

NEW: As of 2026, Forms AB-100, AB-101, and AB-200 have been updated and now have comprehensive instructions preceding the form. Incomplete or old forms will not be accepted.

  • Forms AB-100 and AB-101 have updated instructions. No changes were made to the contents.
  • Form AB-200 has been revised to allow for a more streamlined renewal process that does not require licensees to provide new Forms AB-100 and AB-101 each year if the paperwork from the previous license year is still accurate. 
  • Form AB-200 now includes Appendix A, which creates a standardized listing of all persons involved in the applicant business and an easy way to identify when personal information has changed.

NEW: As of 2026, the Wisconsin Department of Revenue has updated Form CTV-100, Cigarette, Tobacco, and Electronic Vaping Device Retail License Application, to ask applicants to attest to compliance with Wisconsin's Electronic Vaping Device Directory. Incomplete or old forms will not be accepted.

  • REMINDER: As of 2024, the Wisconsin Department of Revenue has updated the Cigarette, Tobacco, and Electronic Vaping Device Applications due to Wisconsin Law changes in 2023 Wisconsin Act 73.  

NEW: Ordinance 02-2025 and Ordinance 10-2025 created the Outdoor Entertainment Permit.  An Outdoor Entertainment Permit is required when the following types of entertainment are provided, wholly or in any part, including fixtures, attendees, and sound systems, outside of a fully enclosed permanent structure:

  1.  Amplified or non-amplified music, including live singers, musicians, bands, or orchestras, recorded music, karaoke, juke boxes, live DJ’s, background music, radios, and the like.
  2. Audio-visual broadcasts, including the presentation of live televised events such as a sporting event, television shows, movies and the like.

The below individuals hold current Direct Sellers, Transient Merchants, and Solicitors licenses in the Village of Germantown. 

License holders are required to carry a copy of their license on their person; each license bears a raised Village of Germantown seal, the license holder's name, and the name of the company they represent. 2025 licenses and dashboard displays are printed on neon blue paper. 

NEW: Ordinance No. 12-25, adopted by the Germantown Village Board on May 19, 2025, amended Section 12.08 of the Germantown Municipal Code related to the regulations of Direct Sellers, Transient Merchants, and Solicitors. Applicants that were issued Direct Sellers, Transient Merchants, and Solicitors licenses on or after May 20, 2025, are subject to the enclosed regulations. (*License holders subject to Ordinance No. 12-25 are marked with an asterisk in the list below.)

As of 03/02/2026: NONE
